Per state regulations: we only issue birth and death certificates for events that have occurred within the last 5 months & 28 days in Lane County.

Birth Certificates for Infants under 5 Months and 28 Days

Lane County Vital Records is only able to provide certificates for infants born in Lane County in the past 5 months & 28 days.

**Please note that the hospital will never order birth certificates from the county or state, it is up to the parents to do so**

Ordering options:

US MAIL: Requests submitted by mail should include a complete order form, a copy of valid photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address.

DROP OFF:
Orders can be dropped off in our DROP BOX outside the Vital Records office on the fifth floor of the Health & Human Services building (151 W. 7th Ave. Suite 100-1A). Requests submitted in our drop box should include a complete order form, a copy of valid photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address. Cash can be included in the envelope; it is a secure box.

Orders can be EMAILED to us at

Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address.


IN PERSON:
Orders may be placed in person at our Vital Records office on the fifth floor of the Health & Human Services building (151 W. 7th Ave. Suite 100-1A). Our walk-in hours are Monday – Friday from 10 AM to noon and from 1 PM to 4 PM.

Birth Certificates for Individuals older than 5 Months and 28 Days

Birth Certificates for individuals born in Oregon more than 5 months & 28 days ago must be requested through Oregon State Vital Records.

Death Certificates within 5 Months and 28 Days from Date of Death

Lane County Vital Records is only able to provide Certificates for deaths which occurred in Lane County within 5 months & 28 days from date of death.

Death Certificates for over 5 Months and 28 Days from Date of Death

Certificates for deaths which occurred in Oregon more than 5 months & 28 days from date of death must be requested through Oregon State Vital Records.

Fees

Birth and Death Certificates Fees are $25.00 per certificate, as of January 1, 2018.
