ABOUT US

The mission of the Community Health Centers of Lane County, a federally qualified health center (FQHC), is to improve the health and wellness of our community through access to affordable, holistic healthcare.

We have adopted a model of primary care called Patient Centered Medical Home. This model of primary care prioritizes the patient, who has increased access to other services  beyond general medical care at our clinics. We coordinate our patients' health care across the community, among specialists, emergency and hospital visits, diagnostic testing services, and other services.

Patients at Community Health Centers of Lane County are assigned to a care team, consisting of many different types of professionals. Each team member adds different skills and knowledge to support the health of our patients.

We believe healthy people are the foundation of healthy communities. Each of our clinics provides superior, affordable primary care for the whole family. We offer compassionate care for infants, children, adolescents, adults and seniors.

STAFF

At CHCLC, we have a dedicated staff of more than 200 people who work tirelessly to ensure that everyone in Lane County has access to complete medical care.  Our teams include bilingual (English-Spanish) medical providers, nurses, patient care coordinators, access specialists and front desk staff.

Briana received her bachelor’s degree in Community Studies and a minor in Education in 2001. She earned her graduate degree in Nursing from Columbia School of Nursing in 2008.

 

Briana has a certification by AANP. She has worked as a nurse practitioner in community health for the past eight years providing primary care, chronic disease management, health promotion and disease prevention for adults. Briana joined our practice in 2015. Briana grew up in Eugene and she excited to come back to the Eugene community after being away for a while.

 

She enjoys reading, music and spending time with her family. Briana has a passion for working in community health and providing high quality health care to her patients.

Karen is a Physician Assistant; she earned her Master of Science degree from Western University of Health Sciences in 2010. In 2005, Karen earned her paramedic degree and afterwards earned a bachelor’s degree in General Science from Oregon State University in 2008.

Karen has specialized experience in Obstetrics, Gynecology and Gastroenterology. She joined our practice in January 2015.

As a native Eugenean, Karen is passionate about providing high quality care to the community and takes pride in her ability to connect and communicate with patients. Karen and her husband have three children and enjoy going to the coast, jogging, hiking; and spending time with her many family members that call Eugene home.

Dr. Castañeda graduated from the University of Notre Dame with a bachelor's degree in Spanish and was awarded her medical degree from the Creighton  University School of Medicine. She completed a residency at the Nicklaus Children's Hospital. She is a member of the American Academy of Pediatrics. 

Dr. Castañeda joined our practice in 2018. She grew up in southern California but has moved throughout the country and enjoys experiencing new locations. She loves college football and in her spare time, Dr. Castañeda likes to curl up to a good historical fiction book.

Dr. Colgrove graduated from Vanderbilt University in Nashville, TN, where he completed his medical school and pediatric training. His primary interest is in general pediatrics and has a special affinity for and training in nutrition (“Food as Medicine”) as well as genetics. He spent his first four years after residency on the Navajo reservation- a unique and deeply meaningful experience for him. Dr. Colgrove is board-certified and has been practicing pediatric medicine for over 25 years.

Dr. Colgrove moved to Eugene in 2013 and is married to his wife who grew up in a small village in Kenya. They have two wonderful children in college in Seattle. The past three years of both his personal and professional lives have been transformed by learning about nutrition and Lifestyle Medicine. Applying this new-found science has brought new insights in the understanding and application of disease prevention and health promotion.

Dr. Colgrove joined our practice in 2023, and is excited to be at Community Health Centers where he can really focus on the health and happiness of his patients and community.

Dr. Colgrove enjoys learning about other cultures, playing board games, the outdoors and learning about nutrition and cooking.

Nelya Drofyak Curry PA-C, MPAS earned a bachelor’s degree from Willamette University then completed Physician Assistant studies from Oregon Health & Science University School of Medicine. She is board-certified by the National Commission on Certification of Physician Assistants (NCCPA). She is also an active member of Oregon Society of Physician Associates (OSPA) and American Academy of Physician Associates (AAPA).

PA Curry joined our practice in 2023. She is passionate about improving health outcomes and eliminating barriers to care for underserved patients. Her medical areas of interest include neurology, sexual and gender health, opioid dependence, and preventative medicine. 

The aspect of patient care that she enjoys the most is empowering patients to take an active role in their healthcare.

She immigrated at a young age from Soviet Ukraine, growing up in Salem, OR. She is a native Ukrainian and Russian speaker. She enjoys spending time with her family playing soccer, anything outdoors, gardening, and learning about other cultures. One of her goals is to hike the whole Pacific Crest Trail before the age of 50.

Dr. Dear has a Bachelor of Science in physical therapy from East Carolina University, and a master’s in public health from the University of North Carolina-Chapel Hill with a concentration in health behavior and health education. She earned her medical degree at the University of North Carolina Chapel Hill and completed her residency in family medicine at Duke University Medical Center.

Dr. Dear joined our Community Health Centers of Lane County practice in 2024.

Dr. Dear grew up in North Carolina and moved to Oregon in 2015. In her free time, she enjoys sewing, quilting, gardening, and baking.

Laurel Decou earned her masters of nursing degree from the University of California, San Francisco in 2022. She is a member of the American Association of Nurse Practitioners.
Laurel joined our practice in 2023. She speaks some Spanish and French. Laurel grew up in Oregon, mainly in the Eugene area. After living out of state for many years, she is excited to be back here with her family, including her husband and their two kids. She loves books and is big fan of the library.  The rest of her time she’d rather be outside gardening, hiking and camping.

Ellie received her bachelor’s degree in Psychology from the University of Texas. She earned her master’s degree in Counseling at Arizona State University and her doctorate degree in Counseling Psychology at the University of Oregon.

Ellie is a Behavioral Health Consultant at the Brookside Clinic. She provides assessments, brief therapy for psychological issues such as anxiety, depression, PTSD, trauma, etc., as well as behavioral assistance with physical health conditions. Ellie has special training in smoking cessation and behavioral strategies for pain management.

She is a Licensed Psychologist and does not prescribe medication. Ellie joined our practice in 2012. In her leisure time, Ellie likes to read, visit with her friends and family, play with her dog, garden, and explore the Northwest.

Enyo earned her Bachelor of Science degree in Human Development and Psychological Services from Northwestern University in 2002. She completed her Health Sciences post-baccalaureate from the University of Oregon in 2004 and earned her graduate degree in Nursing from Yale School of Nursing in 2007. Enyo is certified by the Pediatric Nursing Certification Board. Enyo joined our practice in 2016.

 

Enyo’s first job out of graduate school was in a pediatric specialty clinic in San Francisco for 3.5 years working in pediatric endocrinology treating children with Type 1 & 2 diabetes, growth hormone, thyroid, and pubertal conditions. She went on to take a position at The Children’s Hospital of Philadelphia in the Endocrine department for 5.5 years treating a rare condition in children that were admitted to this hospital from all over the country and internationally.

 

Enyo grew up in Oregon and attended high school in Eugene. After marrying and having a baby Enyo and her husband decided to be closer to family. She enjoys spending time with her family, outdoor activities and sporting events. Enyo is happy to be back in Oregon, working to keep kids healthy.

Dr. El-Hashemy earned his doctoral degree in Naturopathic Medicine at the Canadian College of Naturopathic Medicine. He earned his masters of Education from the University of Toronto, and a bachelor of Medicine & Surgery from Cairo University. Dr. El-Hashemy has over 13 years of experience in naturopathic medicine, primary care and pediatrics. He speaks English and Arabic. Dr. El-Hashemy joined our practice in 2022.

Dr. El-Hashemy was born and raised in Giza, Egypt. He is passionate about primary care and pediatric mental health. He is also an advocate of evidence informed medicine in a manner that neither rejects conventional medicine nor accepts alternative therapies uncritically. Tom was born in Australia. He grew up in Eugene and attended French Immersion High School. After college, he lived in France and Spain where he taught English and learned languages. Dr. Murison graduated from the National University of Natural Medicine in Portland Oregon in 2010 and has been practicing for over 10 years in various facilities throughout Oregon.  He joined our practice in September, 2021.
He has a passion for family medicine, natural medicine, LGBTQ+ care as well as gender affirming care. 
Dr. Murison was born and raised in Mountaindale Oregon, then moved to Beaverton with his husband before deciding to make the move down to Eugene to join the Lane County community.  Dr. Murison enjoys taking advantage of the vast outdoor spaces that Oregon has to offer by hiking, biking and skiing.  He also enjoys and actively participates in local volleyball groups in the area.

Colleen obtained her Diploma in Nursing from Aultman Hospital School of Nursing in 2001 (now Aultman College). She earned a Bachelor of Science in Nursing in 2019 and a Masters of Science in Nursing degree  from Grand Canyon University in 2021. In 2023, Colleen earned a Post-Master’s Family Nurse Practitioner Certificate from the University of Cincinnati.

Colleen is certified with the AANP and has previous experience working as a nurse in the emergency department and cardiac intensive care unit. She joined our Community Health Centers of Lane County practice in 2024.

Prior to becoming an RN she was a paramedic/firefighter for the community she grew up in and met her husband. After they got married, she became an RN, however she missed the community involvement she had as a paramedic/fire fighter and wanted to return to her roots. She has a passion in caring for the underserved population and is excited to provide care to those who need it most.

Colleen grew up on a farm in NE Ohio, then Charlotte NC for 14 years. In her free time, she enjoys riding her horses, hiking and reading.

Jason initially received his Bachelor of Arts degree in sociology/anthropology from St. Olaf College in Northfield, Minnesota. He then received his Bachelor of Science degree in nursing from the College of St. Scholastica in Duluth, Minnesota. Upon graduation, he moved to Fargo, North Dakota and worked as a registered nurse for three years on a medical/surgical floor gaining valuable experience in the treatment of a wide variety of illnesses and diseases. While working as a registered nurse, Jason completed his Master of Science in Nursing degree in Family Medicine from the College of St. Scholastica. He is board-certified as a nurse practitioner through the American Nurses Credentialing Center (ANCC) in Family Medicine. He is also a member of the American Association of Nurse Practitioners (AANP). Jason joined our practice in 2015.

 

Jason expresses “I am thankful for the opportunity to work for Community Health Center of Lane County. I enjoy the challenge and satisfaction of managing both acute and chronic illness as well as promoting health and well-being across the lifespan in family medicine.”  In his spare time, Jason enjoys reading, hiking, and competing in running and triathlon races.

Kristan received her bachelor’s degree in Human Physiology in 2021 and earned her Master of Physician Assistant Studies degree at OHSU in 2013. She is a member of the OSPA and AAPA. Kristan has worked in primary care, urgent care, spinal injury hospital and long-term care facility.
She joined our practice in 2022. Kristan speaks some Spanish, and is hoping to become fluent so she can see Spanish-speaking patients on her own, without interpreting assistance.
Kristan grew up in Thurston, Springfield and graduated from Thurston High School. She lived in England for 6 months while she worked at a spinal injury hospital. Her family started fostering dogs in 2021 and has helped 23 dogs so far. She loves to bake and celebrate anything! Dr. Turner earned her medical degree from the University of Missouri School of Medicine in 2011. She completed a residency at John Peter Smith Family Hospital in Fort Worth, Texas in 2014. Dr. Turner had training in rural medicine and obstetrics. She is a member of the American Academy of Family Physicians and is board certified in Family Medicine by the American Board of Family Physicians.

Dr. Turner joined our practice in 2023. She is originally from the Midwest but discovered the beautiful Pacific Northwest and ultimately decided to move here long-term. She enjoys travel, simple living, reading, hiking, and camping. In her time off, she can frequently be found hiking through the woods or near the beach, visiting a local festival, or perusing a local bookstore.

Scott is a Physician Associate at the RiverStone Clinic working in Family Medicine. He graduated Physician Associate school at Oregon Health and Science University in 2005. Scott is board certified in Lifestyle Medicine. He joined our practice in 2024. Scott is a strong Spanish speaker.

Scott was born and raised in Eugene, Oregon. He shares, “I was a professional bicycle racer, and then moved my family to the Portland area for school to become a Paramedic, and then ultimately to become a Physician Associate. I learned about Lifestyle Medicine when my own health challenges lead me to study the science of Nutrition and the benefits of lifestyle change. I am passionate about improving the health of my patients, family and friends utilizing therapeutic lifestyle change, including using food as medicine. I teach free classes on Nutrition and Lifestyle Medicine with some of my physician colleagues, both online and in person. I also enjoy bicycle riding, college sports, and spending time with family and friends. I am excited to join the Community Health Centers of Lane County team.”

Dr. Walter received his doctoral degree in naturopathic medicine from Bastyr University in Seattle and completed a residency at the Bastyr Center for Natural Health. Dr. Walter specializes in naturopathic healthcare. He has a special interest in using nutrition, dietary supplements, herbal remedies, meditation, massage and spinal manipulation as part of his primary care practice. Dr. Walter joined our practice in 2013 and is the Associate Medical Director for the Brookside Clinic.

Ellis Whalen is a nurse practitioner with the Community Health Centers of Lane County at the Brookside Clinic. She is a board-certified family nurse practitioner through the American Nurses Credentialing Center (ANCC). Ellis received her Doctor of Nursing Practice (DNP) graduate degree from Regis College in 2022, her bachelor’s degree in Biology from Texas A&M University in 2014 and a Bachelor of Science degree in nursing from Denver College of Nursing in 2016. Ellis moved from Denver, Colorado to Eugene in early 2022 and is excited to serve the community as a provider with the Community Health Centers of Lane County. She is familiar with the Community Health Centers as she completed a year of her family medicine clinical rotations at our Delta Oaks Clinic. Ellis has worked as a registered nurse and clinical educator with diverse experiences in pediatrics, medical surgical nursing, rural community health, and long-term care nursing. She is passionate about patient empowerment, education, and the provision of holistic, patient-centered primary care services for patients across the lifespan. Ellis is an active member of the Oregon Nurses Association and the American Association of Nurse Practitioners. In her spare time, Ellis enjoys hiking, long-distance running, reading, and spending time with her partner and two dogs.

Jodi earned master degrees in both Counselor Education and Nursing from the University of Florida/Gainesville.  She is nationally certified in Pediatrics since 2006 through the American Nurse Credentialing Center. Jodi is a member of the National Association of Pediatric Nurse Practitioners. She is very passionate about pediatric nutrition and enjoys educating her patients. Jodi joined our practice in 2016.

Jodi grew up in Brooklyn NY, and has lived in Fort Lauderdale, FL, Asheville, NC, Bend and Portland.  In her free time, she enjoys urban gardening, trail walking and being outdoors.

Jennifer Adamo, EPDH

Jennifer earned a Bachelor of Science in Dental Hygiene with a minor in business from the Oregon Institute of Technology. She obtained her dental hygiene license in 2009, her expanded practice dental hygienist permit in 2015, and her certification as a certified oral systemic educator in 2022. She is a member of several associations, including the Oregon Dental Hygiene Association, the National Network of Healthcare Hygienists, the National Network for Oral Health Access, and the American Dental Hygiene Association.

Jennifer feels incredibly fortunate to be able to serve both adults and children in the community where she grew up. She has an immense passion towards preventative dentistry, improving oral health outcomes, and educating patients regarding the connection between systemic and oral health.

Outside of the CHCLC, Jennifer loves spending time with her husband and her family, attending concerts, traveling, being active, and supporting the Oregon Ducks football team.

Melinda Davis, RDH, EPDH

Melinda Davis
Melinda (Mindy) Davis started her career in 2005 as a dental assistant for the Air Force Reserves. In 2011, she decided to return to school to complete her Dental Hygiene education. She graduated from Lane Community College, where she earned her Associates Degree of Applied Science in Dental Hygiene including the Expanded Practice Permit in 2015. She joined Community Health Centers of Lane County in October of 2021 working with the dental outreach program working as an Expanded Practice Dental Hygienist, where she has found her passion being able to serve the children in our community.

Mindy has worked in private practice since 2015 as a Registered Dental Hygienist, and continues to work part time serving her patients of all ages. She has a passion to provide exceptional dental care along with providing information about the importance of dental care and building a connection with all those around her.

In her free time, Mindy loves to spend time with her husband Jason and their two dogs (Kimber and Axel) and relax with their cat (Whiskers). She loves to travel, spending time with family, exploring the outdoors and staying active fishing, boating and camping on the rivers.     




Kellie Green, RDH, EPDH, BS

Kellie’s education includes a Bachelor of Science in Exercise Science, from Pacific Lutheran University, after which she worked in the area of Cardiac Rehabilitation, before taking time to travel abroad, exploring a bit of our world.

Kellie received her Associates Degree as Registered Dental Hygienist from Lane Community College, and joined the Community Health Centers of Lane County in 2011.  She loves the work the Dental team does, serving the children of our community, promoting the importance of oral health, and providing preventative services.

Alongside her work with the CHCLC, Kellie also works in a general, family practice dental office, with patients of all ages.  

Kellie grew up in Eugene, on the outskirts of town, and enjoys many of the outdoor opportunities Oregon has to offer. In the winter you’ll find her with her family in the mountains on either cross country or downhill skis.  In the summer, they’ll be camping in one of Oregon’s many beautiful places, backpacking through the mountains, or reaching for mountain tops with their local climbing club.

Kathleen Grover, RDH, EPDH

Kathleen received an Associate of Science degree in Dental Hygiene from Lane Community College in 2006. She is a member of the Oregon Dental Hygiene Association & American Dental Hygiene Association.

Kathleen joined Community Health Centers of Lane County in 2007, and also has worked since 2006 for a pediatric dental office. Kathleen has lived in Eugene for 21 years.

Emily Miller, EPDH

Emily received an Associates of Art degree in 2004 and a Associates of Applied Science degree in Dental Hygiene in 2006. She holds a State Certified School Based Sealant Program certification and participates in annual continuing education opportunities at the Oregon Dental Conference.

Emily speaks and writes Spanish. She joined Community Health Centers of Lane County in 2006 as an extra help employee and now is a permanent employee.

Emily enjoys helping the “littles” of our world to grow and have health. She also loves to be in motion with snowboarding, playing roller derby, skating outdoors and hiking in the woods. Lane County is beautiful and Emily feels so lucky to live here. You will find her in the woods when she is not at work.

Tia Morehouse RDH, EPDH

Tia graduated from Lane Community College with an Associate in Applied Science, Dental Hygiene in 2017 and received her Expanded Practice Dental Hygiene Permit shortly after graduation.  Tia started working for the Community Health Centers of Lane County in February of 2022.  She comes to us with experience working in a similar dental outreach program at the South Lane Children’s Dental Clinic, where she was the lead hygienist for their school based sealant program since 2017.  The favorite part of her job is being able to get the children in the community the dental help they need. Tia moved to Eugene, from Grants Pass, in 2010 when her husband got a firefighting job at the Eugene Springfield Fire Dept.   During her spare time she loves to spend time with her family, her husband and two kids, working out, being outdoors and traveling.   

Sandra O’Keefe, EPDH

Sandra started working for Lane County as an Expanded Function Dental Assistant for the Lane Community College Dental Clocktower Clinic in 2008.  After graduating from the Dental Hygiene program at LCC, Sandra started working for Lane County as a Registered Dental Hygienist then expanded her functions as an Expanded Practice Dental Hygienist.  Sandra was born in Eugene, but grew up in Southern Oregon.  She moved back here in 1995 to raise her children and go to school to become a Dental Hygienist.  She loves rafting and hiking and being a Grandma!  Sandra’s favorite part of working for Lane County is helping children in Lane County with their oral health.

Alicia Riedman RDH, EPDH

Alicia moved to Oregon in 2001, from West Virginia, where she was a massage therapist for 10 years, with her husband and 3 children.

Alicia graduated from Lane Community College with an Associate in Applied Science, Dental Hygiene in 2006. Alicia shares, “As a third generation dental professional, I am honored to have worked for Community Health Centers of Lane County as an outreach Dental Hygienist since 2006. After participating in the Oregon State Smile Survey in 2007, I saw the tremendous need for children to receive dental prevention services in an outreach program.”

Alicia worked part -time at Community Health Centers and part-time in a private Dental Practice from 2006-20I0. She received her Expanded Practice Dental Hygiene permit in 2010 and a full-time position at Community Health Centers of Lane County and is now a Senior Expanded Practice Dental Hygienist.

Alicia holds a current Dental Hygiene position on The Oregon Board of Dentistry, since 2015, as well as many state and national committees, working to provide the highest level of dental care to the state of Oregon. She is on the Western Regional Examination Committee as a local anesthesia clinical examiner, and is an active member of American Dental Hygiene Association, where she also served on the ODHA Government Relations Council.

Alicia loves spending time with her family, as they all live locally, and she especially enjoy the many hours she gets to spend with her grandchildren. In her spare time she loves kayaking and gardening, especially fresh vegetables.
