Just found out you're pregnant?

If you are pregnant, we can connect you to the Oregon Mother's Care program, which helps you complete an Oregon Health Plan application and get you approved faster than if you applied on your own. OHP prioritizes all pregnant clients and they are processed much more quickly.

We will assist you with a referral to WIC for nutritional education and assistance with vouchers for food necessary to have a health pregnancy and healthy baby. Information about other community resources is also available.

The fastest way to get support from Family & Child Health is to complete our request for pregnancy and parent support. After completing this form, a member of our team will reach out as soon as possible. Explore our programs or reach out to a team member to learn more.

How long are the visits?

Personal Nurse Visits

  • Typical nurse visits last 1-1.5 hours, all in the comfort of your home.
  • Family and caregiver education and skill building on how to be the best parent you can be.
  • Nurse Case Management including referrals & support to access resources tied to your family’s goals.

Family Support Specialist Visits

  • Typical visits last 1-1.5 hours, all in the comfort of your home.
  • Family Support Specialists empower and support parents in building nurturing relationships with their child, learn about child development, and access resources in the Lane County community.

Can you help me get housing?

We will support you in navigating available community resources towards housing, including application support and building your self-advocacy skills, but we do not have special access to housing directly.

My child needs home health services. Can you help?

Family & Child Health does not offer home health services, such as tracheostomy, IV, ostomy, or wound care. Please contact your health insurance company or CCO for support in accessing those services. Trillium Member Services at 1-877-600-5472 ; or PacificSource Customer Service at 800-431-4135



This health center receives HHS funding and has federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

LCPH services are available regardless of age, race, color, sex, religion, national origin, physical or mental disability, sexual orientation, gender identity, and marital status. LCPH facilities are wheelchair accessible. LCPH materials are available upon request in alternative formats such as: large print, Braille and other languages.
